Sealing Sunny Smiles Across Florida
This program is part of a statewide oral health plan to promote evidence-based prevention strategies in order to achieve optimal oral health for all Floridians.
Eligibility
Children ages 3-20 are eligible for dental treatment at the Florida Department of Health in Baker County. Children who are 2 and under are referred to a children’s dentist who has the special equipment needed for a child’s small mouth. We accept MCNA, Liberty, and Dentaquest Medicaid plans. Patients who are not eligible for Medicaid are charged for services on a sliding fee scale according to income.
Dental services include cleanings, x-rays, fillings, extractions, and sealants.
School-Based Sealant Program
DOH-Baker participates in the School-Based Sealant Program at many Baker County schools.
Parents and guardians will need to complete a consent form packet for each student participating in the program. You can complete the online form or submit a paper form to your child’s teacher or school nurse.

Initiation of Services Form
An initiation of services form establishes the legal and clinical agreement between a client and a provider. It grants consent for routine treatment or care, authorizes the safe handling of medical or personal data, and outlines financial and administrative policies before care begins.
