Eligibility
Children ages 3-20 are eligible for dental treatment at the Florida Department of Health in Baker County (DOH-Baker). Children who are 2 and under are referred to a children’s dentist who has the special equipment needed for a child’s small mouth. We accept MCNA, Liberty, and Dentaquest Medicaid plans. Patients who are not eligible for Medicaid are charged for services on a sliding fee scale according to income.
Dental services include cleanings, x-rays, fillings, extractions, and sealants.
Dental School Bussing Program
This program provides dental care to children ages 3-10 who are on Medicaid or any other need-based programs and are unable to obtain dental care. Eligible students are bussed to the DOH-Baker dental clinic during school hours for comprehensive dental care.
At the beginning of each school year, students receive a dental health history and consent packet for a parent or guardian to sign up for the program. The school nurse will collect permission slips and coordinate the visits with the dental program staff.
Students will be bussed over in groups of five or six for the dental visit and can work on schoolwork while waiting for their appointment.
The dental department will send a letter home with each child explaining which dental procedures were completed and what treatment is still needed.

School-Based Sealant Program
DOH-Baker offers dental sealants to elementary, middle, and high school students in Baker County.
Completing the consent form will allow for your child to receive an assessment/screening by a licensed dental hygienist, along with appropriate treatment that may include a dental cleaning, sealants, fluoride varnish, and oral hygiene instructions. Services will be provided at your child’s school.
