The Baker County Community Health Improvement Plan (CHIP) was developed as a result of the collaborative efforts by Baker County employees, expert consultants, health care leaders, public health professionals, and other vested stakeholders that make up Healthy Baker, the county’s formalized leadership council for health improvement. The purpose of this plan is to describe the short, mid, and long-term strategies and activities related to the targets associated with each community health indicator and to establish priorities and objectives over the next four years.
