Baker County Community Health Improvement Plan (CHIP)
The Baker County Community Health Improvement Plan (CHIP) was developed as a result of the collaborative efforts by Baker County employees, expert consultants, health care leaders, public health professionals, and other vested stakeholders that make up Healthy Baker, the county’s formalized leadership council for health improvement.
This Plan is designed to address weaknesses and build upon strengths identified in the community health assessment and provide a clear course of direction for the future of Baker County’s Health. The purpose of this plan is to describe the short, mid, and long-term strategies and activities related to the targets associated with each community health indicator and to establish priorities and objectives over the next four years.

Community Health Assessment (CHA)
The Community Health Assessment (CHA) was put together following a two-year long process of collecting and analyzing data, engaging and mobilizing key constituents, and developing multiple reports to depict the state of health in Baker County. This collaborative process has resulted in both quantitative and qualitative methods to systematically understand health within our community. It has informed our staff and our community regarding forces of change, community strengths, and opportunities for improvements to affect the quality of life for Baker County residents.

Strategic Plan
The Strategic Plan outlines our purpose, the priorities for the year, and how we will deliver them for the benefit of the public. This Plan is a living document that builds on what we have learned and experienced as a agency over the years. The Plan will continue to be refined and perfected as we deliver health services to the public and continue to be the medical home for many individuals and families in this community.
The Strategic Plan represents a challenging work program for the upcoming implementation period, one that will deliver tangible benefits, offer the highest quality services to our community, and fulfill our role in the coordination of local public health functions.
