The Florida Department of Health and Ed Fraser Memorial Hospital are currently working together to complete a Community Health Assessment of Baker County. Community participation is crucial to the Community Health Assessment, and we value all input about health and well-being in Baker County. Participation in the survey will help us to gather the information that will inform our Community Health Assessment and ultimately help the Florida Health Department in Baker County and Ed Fraser Memorial Hospital identify areas where improvements can be made to the health and well-being of Baker County residents. Any Baker County resident age 18 and up can complete the survey.
