Obtain a Copy in-Person

Death Certificates are available for deaths occurring in the state of Florida from 2009 to present. Any death prior to 2009 must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville or through VitalChek®.

You will need the following information:

A a valid driver’s license, passport, state-issued ID, or military identification card must accompany all requests.

You must be a family member or legal representative when requesting cause of death.

Death certificate addendums and corrections

For addendums and corrections to a Florida Death Certificate due to demographic errors, Form DH 524 (Application for Amendment) and DH 433 (Affidavit of Amendment) must be submitted to the Bureau of Vital Records. Addendums and corrections for medical/cause of death changes require a physician or medical examiner to file form DH434A (Affidavit of Amendment).
