You will need the following information:
A valid form of photo identification such as a driver’s license, state-issued ID, passport, or military ID. All identification must be valid (cannot be expired.)
You will also need to complete a Birth Certificate Application Form, including the person’s full name, date of birth, county of birth, mother’s full name, including maiden name and father’s full name.
